Please note that from 29 June 2019, if a PPI check confirms you are eligible to make a complaint, we will raise this on your behalf. We will then confirm this by email/letter and ask that you complete one of our questionnaires so that we have all of the information we need to complete our investigation. Failure to complete this when requested,will result in us only being able to process your complaint based on the information we have.
